Camp Forms

The following forms must be filled out before your child can participate in camp. Massachusetts law requires that each camper provides documentation of a recent physical (within 12 months of camp) and immunization record that includes MMR. Physical must be signed by camper's physician.

Emergency Contact Form (mandatory)

This document will need to be completed and returned to the Camp Director with an up-to-date physical for each participant.

Camper Pick-up (mandatory)

This document is used to verify who is allowed to sign campers out at the end of each day.

Health History (mandatory)

This document will need to be completed and returned to the Camp Director with an up-to-date physical for each participant.

Medication Log (if necessary)

This document will be shared with our medical/training staff to oversee any medications that need to be taken during camp hours.

Authorization to Administer Medication (if necessary)

This document will be shared with our medical/training staff and allows them to administer any necessary medication during camp hours.

Epi-Pen Authorization (if necessary)

This document will be shared with our medical/training staff and allows them to administer an epi-pen if necessary during camp hours.
